What Immunizations are required at my school?

Where are the immunization forms located?
You may find the forms by visiting the Student Health and Well Center website and selecting the institution you plan to attend, AUCC SHWC Immunization Records for Compliance. Exemption request forms may be accessed on the Point and Click Portal under the Downloadable Forms tab or on our website.

I just submitted my documents. When will I know if they are approved?
Please allow 10 business days for processing immunization information. If they are not approved by then, please check the messages on your Point and Click Portal to see if you have received a message with feedback from our team.

Can I submit my immunization records via email/fax?
For privacy and security of health information, immunizations cannot be emailed, faxed, or accepted in person. Immunizations can only be uploaded on the Point and Click . Please do not request your doctor to fax your records directly to our office. You are responsible for uploading your records into the Point and Click Portal and entering in all required data.

Where can I get an immunization exemption or waiver?
You can find exemption forms on the under "Downloadable Forms".
You can also find the forms on our website.
Please note: Religious Exemption forms require notarization by an official notary.
Exemption forms are reviewed and approved by doctors in the clinic. Please allow up to 10 business days for review.

What do I do if I don't have a Religious Leader as part of my exemption?
In the event you do not have a religious leader to speak on your behalf for the vaccine/vaccines you are requesting, you may visit the website to obtain a Religious Exemption Affidavit form.
You will need to complete the affidavit and have it notarized. After you have completed the form, you will need to upload it into the along with your Religious Exemption forms for approval by our physician. Once your documents have been approved your requirement status will change from Non-Compliant to Compliant.

Why do I need to be immunized against certain diseases?
The purpose of this policy is:
- To ensure that students are safe and protected against communicable diseases that are preventable and may cause serious illness or death.
- To reduce the likelihood of the spread of preventable diseases or a threatened epidemic on a university campus.

I am not from the state of Georgia, are my requirements the same?
Yes, all requirements are the same.

I am not from the United States, are my requirements the same?
Yes, the requirements are the same. All immunization documents must be submitted in English. If you are unable to obtain certain vaccines that are not available in your country prior to arrival, please contact our office for accommodation by calling (404) 756-1241 or emailing SHWCrequests@msm.edu.

Do my documents have to be in English?
Yes, all documents submitted for immunization review must be in English. If you have immunization documents that are not in English, you must have these documents professionally translated.

Does your office provide vaccines for travel?
We provide travel health visits to review your immunizations and determine what vaccines or malaria-prophylaxis may be necessary based on the country you are traveling to. We do not carry vaccines such as Typhoid, Yellow Fever, Polio, or Japanese Encephalitis in our clinic.

Where can I find my access code?
The access code is sent to your student email address.
If you are an admitted student, and cannot access your school email, please check your personal email received from your institution that was provided to you with the Step-By-Step Instruction Checklist of how to access your email.

What is my username?
Your username is everything that appears before the 鈥淍鈥 sign of your student email.
For example, if your email is john.doe@college.edu your username will be 鈥渏ohn.doe鈥.

I got my insurance through the school, where can I find my insurance information?

What insurance plans do you accept?
无码人妻 Healthcare participates in most major insurance plans.
However, it is the responsibility of the student to check with their insurance provider to see if they are covered at our clinic. If you are an out-of-state student, the insurance coverage needs to include a 鈥減oint of service plan for out of state.鈥
Out-of-state Medicaid is not accepted in Georgia.

Can I be seen at the clinic if I don鈥檛 have insurance?
Yes, you can be seen in the clinic if you do not have insurance. There is no charge for you to see a provider.
Though we do not charge for clinic visits, it is important that students have health insurance to cover services that are not covered by the student health fee (e.g., laboratory services, medications sent to a pharmacy, radiology services, immunizations, etc.).

Services and Costs

How much does it cost to be seen at the Student Health and Wellness Center?
Students pay for medical and behavioral health visits offered at the SHWC through the student health fee portion of their tuition. However, some services that may be included in the visit, such as laboratory services, radiology, immunizations, certain procedures, and some medications are billed to the students鈥 insurance. We do offer some medications for free through our in-house dispensary including many antibiotics and over-the-counter cold and flu medicines.
Though we do not charge for clinic visits, it is important that students have health insurance to cover services that are not covered by the student health fee (e.g., laboratory services, radiology services, immunizations, etc.).

What services do you offer?
Health care visits for acute illnesses and injuries, immunizations, STI testing, women鈥檚 health (including IUDs and Nexplanon placement,) health education, primary healthcare, mental health visits (with psychiatrists and psychologists), and allergy injections.

Can the AUCC Student Health and Wellness Center act as my Primary Care Provider?
Yes.

What do I do if I have billing-related issues?
Questions about billing should be directed to our billing department. Most bills received by students are from LabCorp. Please contact LabCorp with questions about these bills.

If I fail to cancel my appointment, will I be charged a No-Show fee?
Yes, you will be charged a $10 fee if the appointment isn鈥檛 canceled within 24 hours.

Will I have to pay for medication that is prescribed by my healthcare provider?
The AUCC-SHWC will prescribe some medications that are dispensed at the AUCC-SHWC at no fee. Some medications are prescribed, you will pick up at a pharmacy and you may incur a fee.
Registration/Holds

Will a hold be placed on my student account if I do not comply with the immunization
policy?
Yes. Students who are not in compliance with their Institutions immunization policy will have a Medical Hold placed on their student account until they have met the requirements. The Medical Hold may prevent a student from receiving a housing assignment, moving into the dormitories, accessing enrollment information, course data, academic transcripts, and other institution services.

What do I do if I have a Medical Hold?
If you have a Medical Hold on your account, please check the Point and Click to verify which immunizations you are missing. You can see which items are missing by checking your Medical Clearances tab.

I just submitted my documentation. When will I know they are approved?
Please allow up to 10 business days for processing immunization information. If they are not approved by then, please check to see if you have any messages within your Point and Click patient portal.

How long does it take to remove a medical hold after I have submitted my records and
they have been approved?
Please allow at least 24-72 business hours for the information to be updated and reflected in your banner account.

How can I make an appointment?
You can make an appointment to seen by accessing the Point and Click and scheduling online or by phone 404-756-1241.

What if I need to cancel or reschedule an appointment?
Notify the AUCC-SHWC as soon as possible, ideally 24 to 48 hours in advance.

What should I bring to my appointment?
Bring your student ID, insurance card, a list of medications, medical history and any questions or symptoms you want to discuss

What if I am late for my appointment?
If you鈥檙e more than 15 minutes late, and the healthcare provider can鈥檛 see you, you will need to reschedule your appointment.

How long does an appointment usually last?
Appointments typically last 15-45 minutes, depending on the reason for your visit.

How do I get my test results?
Your healthcare provider will either contact you by phone or send you your test results through the PnC portal.
- What should I do in case of a medical emergency?
- Call 911 or go to the nearest emergency department if your illness is severe or you
are at high risk instead of waiting for an appointment at the AUCC-SHWC.
- Seek medical attention if you have any of the signs below:
- Trouble breathing
- Persistent pain or pressure in the chest
- New confusion
- Inability to wake or stay awake
- Pale, gray, or blue colored skim, lips, or nail beds, depending on skin tone
- Call 911 or go to the nearest emergency department if your illness is severe or you
are at high risk instead of waiting for an appointment at the AUCC-SHWC.
- What if I need a medical appointment after normal office hours?
SHWC has extended hours of operation on Monday and Wednesday until 8pm. Students can contact SWHC at 404-756-1241 to schedule an appointment.
- What to do if I have a Non-Life-Threatening Medical Problem?
- Student should contact SHWC after hours line for guidance at 404-756-1241 OR
- Student may seek medication attention at nearest urgent care facility
- Symptoms that can be evaluated and treated at an urgent care clinic include:
- Fever without a rash
- Vomiting or persistent diarrhea
- Abdominal pain
- Wheezing or shortness of breath
- Dehydration
- Moderate flu-like symptoms
